Free Whitening Program Requirements
To be eligible for the program as a patient of Advanced Dental Concepts, you must meet the following requirements:
- Complete prescribed new patient exam, x-rays and a cleaning with Advanced Dental Concepts.
- Have all tooth decay removed within one year. This is required in order to prevent complications that may arise with unhealthy teeth and gums.
- Stay current with your dental account.
- Be at least 18 years old, not pregnant or nursing.
*Advanced Dental Concepts reserves the right to refuse on-going whitening treatment if deemed necessary based on patient health conditions, misuse, abuse, or any other factor deemed necessary to void the offer. Lost or destroyed custom trays will be replaced at cost to patient. Patients who lose eligibility will then need to complete 2 prescribed hygiene care appointments and/or restore all decay to be reinstated in the Free Teeth Whitening for Life program.
